How do I know which custom declaration form I need and when do I need to use it?

All international mail weighting more than 1 lb. and/or $400.00 in value requires a custom declaration form.

Use PS form 2976 (green) for mail that is under $400.00 and weighs from 1 lb. to 4 lbs.
Use PS form 2976A with plastic pouch for mail that is over $400.00 and weighs over 4 lbs. or exceeds maximum combined length, width and height of 36 inches.
